What Can You Expect?

We are looking for a dedicated invididual to join us in the role of Patient Transport Liaison Officer, working 37.5 hours per week, to play a vital role in ensuring smooth patient discharges and excellent transport coordination at hospitals in the region.

In this role, you can make a huge difference to patient’s journeys through liaising with other healthcare professionals at different levels, both internally and externally.



Summary Of Main Duties Include:

Be the go-to patient transport expert on-site in the Discharge Lounge and Transport Office.
Collaborate with hospital teams to streamline patient flow and prioritise transport needs.
Carry out risk assessments as required.
Represent transport interests at daily bed meetings, aligning plans with critical discharge timelines.
Maintain clear communication between wards, patient flow teams, and transport providers.
Keep patient records accurate and secure, ensuring smooth transitions and compliance with data protection policies.
Provide support to patients, families, and care facilities, ensuring everyone is informed and prepared for discharge journeys.

We are Looking for Great People Who:

Previous experience within non-emergency patient transport, or a similar industry.
An excellent communicator with the ability to build strong and effective relationships with colleagues at all levels.
Proactive and thrives in a fast-paced environment.
Exceptional organisational skills to balance priorities and manage resources effectively.
A compassionate professional dedicated to patient care and confidentiality.
Confident with using technology, updating IT systems and handling sensitive patient data.
Must have a full UK manual driving licence with no more than 6 points.
