Summary
To provide hands on care, planned advice, support, and instruction to patients and where relevant their carers and other professionals, promoting understanding of the aims of their treatment plan / goals and to ensure a consistent approach to patient care. To communicate effectively with all patients including those with mental health needs, and their families/carers using a range of verbal and non-verbal communication tools to progress agreed management plans and treatment programmes ensuring patient understanding of treatment proposals to gain valid informed consent according to organisational policies. To evaluate emergency situations and act accordingly working within procedures and protocols laid down including knowledge of DNACPR protocols. Carry out duties across community settings including lone working and visiting service users in their own homes travelling independently between sites as required.
