Join Our Community Diabetes Service in Harrow

An exciting opportunity has arisen for an experiencedBand 7 Community Diabetes Specialist Nurseto join the Community Diabetes Service in Harrow. The post is based atHoneypot Lane Health Centreand involves working within a well-established multidisciplinary team to deliver high-quality, evidence-based diabetes care across community settings.

The successful candidate will provide specialist clinical expertise to support adults living with diabetes, promoting effective self-management and improving patient outcomes. The role requires both autonomous practice and close collaborative working with primary care, community services, and secondary care colleagues.

In addition, the post holder will contribute to service development initiatives, deliver structured education to patients and healthcare professionals, and support the ongoing improvement and transformation of community diabetes services within Harrow.

• Support the Team Leader in delivering a high-qualityspecialist Community Diabetes Nursing Service, contributing to service planning and delivery in line with Trust objectives.
• Provideexpert clinical assessment, management, and reviewof patients with diabetes, ensuring safe, evidence-based, and patient-centred care.
• Work collaboratively as part of themultidisciplinary team, liaising with primary, secondary, and community care services to ensure coordinated care pathways.
• Lead and support thedevelopment, implementation, and evaluation of specialist nursing interventionsand service improvements.
• Useproject management and clinical leadership skillsto contribute to service development aligned with national guidance, local priorities, and best practice.
• Maintain accurate, timely, and comprehensiveclinical documentation and recordsin line with professional and organisational standards.
• Contribute to the development and review ofclinical guidelines, policies, and procedures, ensuring high standards of care within the service.
