Job Purpose: To ensure that our clients receive appropriate support and a high-quality service at all times.
The Role:
Act as the main point of contact for allocated clients and their families.
Build trusted long-term relationships through regular proactive contact.
Conduct service reviews in line with company standards.
Ensure care plans are personalised and outcome focused.
Identify early signs of dissatisfaction or risk and intervene promptly.
Protect and grow client hours through thoughtful service delivery.
Maintain accurate client records and ensure care plans reflect current needs.
Collaborate with a business development team for smooth client handover.
Work closely with care Professionals to ensure client needs are met.
Create, update and audit your client care plans and assist with digital care planning.
Maintain accurate client records on Home Instead software - People Planner IQ/ACP
Conduct client and Care Professional introductions.

Essential Criteria
Level 3 NVQ in Health and Social Care or equivalent.
Experience in the care sector delivering a wide range of personal care services.
Experience working with MAR sheets and medication in a supervisory role.
Passion for delivering the highest quality care to our clients and helping them to live
independently and happily in their own homes.
Knowledge and understanding of legislation and regulations specific to Health and Social Care.
Good communication skills with the ability to build rapport quickly.
Must be confident to use care management technology including providing support and training to Care Professionals.
Must understand the importance of confidentiality working within current legislation.
Good working knowledge of IT systems with experience of Microsoft Office or Google Suite and virtual communication platforms with the aptitude to learn and adopt new technologies and software where appropriate.

Must have a full driving license and means of transport if required within the territory to visit
clients.
Be organised and flexible to meet the needs of the business.
