Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate (e.g. history-taking, holistic assessment, identifying risk factors, mental health assessments, requesting, undertaking and/or interpreting diagnostic tests and conducting health needs assessments). Challenge practice so that the best and most appropriate treatment is given to the patient, maintaining and monitoring high standards of care, recognising and responding proactively to patients changing clinical needs Working collaboratively, managing a group of patients to deliver high quality skilled, evidence based and patient centred 1. Be fully conversant and wholly accountable for your practice and maintaining your professional registration in line with the Nursing and Midwifery Councils (NMC) Code Recognise the limits of individual competence and knowledge and undertake and complete further training and academic qualifications as relevant to the role and service requirements. Develop a culture of providing and accepting constructive feedback and use this to reflect on your own practice and that of others. Communicate clearly and appropriately with patients, carers/ relatives and all other members of the multidisciplinary team Continuously assess and evaluate the communication, educational and information needs for patients and their families and devise plans to ensure needs are met; ensure instructions/information is understood. Participate in the development and implementation of a robust clinical audit programme as well as participation in the clinical governance agenda in line with the risk management strategy and policy Contribute and participate towards patient engagement and involvement initiatives e.g. patient focus groups and patient information sessions. Contribute to the training needs and gap analysis of the service and provide training and education to meet the service requirements. Act as a clinical role model and demonstrate specialist knowledge and high standards of clinical practice.
