Summary
Main Duties of the Role The successful candidate will: Lead and support the management of patients with diabetes in accordance with current clinical guidelines. Conduct diabetes reviews, insulin initiation and monitoring where appropriate. Provide education and support to patients to promote self-management and improve health outcomes. Support the achievement of Quality and Outcomes Framework (QOF) targets relating to long-term conditions. Deliver a range of general practice nursing services including cervical screening, immunisations, wound care and health promotion. Work collaboratively with GPs and the wider multidisciplinary team to provide high-quality patient care. Maintain accurate clinical records and contribute to service development and quality improvement initiatives. A full job description is available upon request.
