Summary
You will be responsible for providing a safe, efficient and accurate ward based pharmacy service by liaising closely with pharmacists, nursing and medical staff. This role will help you to develop Medicines Optimisation and Patient Counselling skills. Patient Centred Medicines Management Provide ward based medicines management services, including medicine reconciliation, discharge planning, and patient and carer counselling. Act as a key link between ward teams and pharmacy services, helping to resolve medicines related issues and optimise the patient experience. Ensure the safe, effective, and timely supply of medicines to patients across the inpatient cancer wards at Guy's Hospital. Dispensing and Medicines Safety Undertake dispensing, final accuracy checking (as an Accuracy Checking Pharmacy Technician), and issuing of medicines to maintain a safe and efficient service. Complete medication storage audits, including Controlled Drug audits, stock reconciliation, and expiry checks, reporting any discrepancies promptly. Develop and maintain a safe, efficient, and cost-effective medicines supply service to patients. Teamwork, Training, and Development Work collaboratively with nursing, medical, and pharmacy colleagues within the inpatient cancer ward teams as part of a multidisciplinary team. Participate in clinical audits, quality improvement initiatives, and service development projects. Support the training and supervision of rotational pharmacy staff and trainees, contributing to a culture of continuous learning. You will work in a structured environment providing support to the wider team to meet key directorates targets. This will include delivering pharmacy services to a range of patients with complex cancer conditions, supporting and training junior staff. Candidates should have demonstrable competencies in hospital clinical pharmacy practice, excellent communication skills and commitment to pharmaceutical care.
