The challenge

Traditional hospital discharge processes are severely delayed by time-intensive manual documentation requirements. Patients remain on wards for hours while time-pressed doctors complete paperwork. This creates bed shortages across the hospital. The manual process often leads to inaccurate patient details that can compromise patient safety.

The solution

The AI-Assisted Discharge Summary system uses Large Language Models hosted on the NHS Federated Data Platform to automatically extract key details from patient medical records, such as diagnoses and test results, generating comprehensive discharge documentation. After full review and approval by the responsible medical expert, the system facilitates secure information transfer to different care services, reducing administrative burden while ensuring accuracy and continuity of care.

The results

Currently in pilot phase at Chelsea and Westminster NHS Trust.
