The cardiovascular disease modern service framework sets out the government’s 10-year strategic vision and delivery model for the health and care system to reduce premature deaths from heart disease and stroke in England.

It sets 12 priorities for earlier prevention, diagnosis and treatment of cardiovascular, kidney and metabolic risk factors, with a focus on reducing health inequalities and shifting care closer to home.

In both the PDF and the HTML, made the following corrections: in the executive summary, corrected the estimated potential impacts of the CVDACTION Impact Model from 'over 60,000 events avoided' to 'over 57,000 events avoided', 'productivity gains of £1.3 billion' to 'productivity gains of £1.2 billion' and 'health and social care savings of £1.2 billion' to 'health and social care savings of £1.1 billion'; corrected the metric name in priority 3 from CVDP001CVRM to CVDP001CVKM and in priority 8 from CVDP009HF to CVDP003HF. Also made minor spelling and editorial changes throughout to improve readability and interpretation.
