Dairy and milk: model health certificates

Competent authorities of exporting countries should use the relevant model health certificate as a template to create a version exporters can apply for to export dairy and milk to Great Britain, the Channel Islands or Isle of Man.

  1. All old versions of certificates have been removed.

  2. Certificate GBHC416 Model health certificate for dairy products and raw milk that have undergone a lower heat treatment than pasteurisation for human consumption from third countries listed in accordance with GB import authorisation IMP/GEN/2024/01 (Milk-LHT) has been added, the version history and details have been updated accordingly

  3. We have updated certificates GBHC410, GBHX411, GBHC412, GBHC413, GBHC414 and GBHC415. We have also updated the ‘use from’ date for certificates GBHC410, GBHX411, GBHC412, GBHC413, GBHC414 and GBHC415. Use from date for EU changed to 31 January 2024. Use from date for non-EU countries changed to 30 April 2024.

  4. We have replaced all EU certificates ending in E with new certificates that can be used by all countries. EU countries must use the new certificates from 31 October 2023. Non-EU countries can continue using certificates ending in X or start using the new certificates. We will communicate the deadline for non-EU countries switching to the new certificates in due course.

  5. Certificates GBHC064E, GBHC065E, GBHC066E, GBHC067E, GBHC068E, and GBHC069E should be used from 31 December 2023.

  6. Multiple changes have been made: 1. Amended box 1.2.a in certificates GBHC064E, GBHC065E, GBHC066E, GBHC067E, GBHC068E, GBHC069E. A unique notification number (UNN) needs to be included in this box. 2. Corrected certificates GBHC064E, GBHC065E and GBHC069E to version 3.0. 3. Amended the version history notes for certificates GBHC064X, GBHC064E, GBHC065X, GBHC065E, GBHC066X, GBHC066E, GBHC067X, GBHC067E, GBHC068X, GBHC068E, GBHC069X, GBHC069E. The previous version of each certificate will continue to be accepted until 1 April 2023.

  7. We’ve updated certificates GBHC064X, GBHC064E, GBHC065X, GBHC065E, GBHC066X, GBHC066E, GBHC067X, GBHC067E, GBHC068X, GBHC068E, GBHC069X, GBHC069E. See version history for full details of changes.

  8. We’ve updated certificates GBHC064X, GBHC065X, GBHC066X,GBHC067X, GBHC068X and GBHC069X.

  9. The following certificates will come into use on 1 September 2022: 605/2010 GBHC064E, 605/2010 GBHC065E, 605/2010 GBHC066E, 605/2010 GBHC067E, 605/2010 GBHC068E and 605/2010 GBHC069E.

  10. Updated model health certificates GBHC066E, GBHC066X, GBHC067E and GBHC067X.

  11. Added a version history of changes to model health certificates.

  12. Changed the date that some of the forms should be used to 1 July 2022.

  13. Published model health certificate for milk and milk products from New Zealand (GBHC204).

  14. Updated Milk-HTB dairy products certificates GBHC066E and GBHC066X.

  15. Edited the dates for all health certificates that were previously to be used by 1 April 2021. The following certificates will now come into use from 1 October 2021: 605/2010 GBHC064E 605/2010 GBHC065E 605/2010 GBHC066E 605/2010 GBHC067E 605/2010 GBHC068E 605/2010 GBHC069E

Dairy and milk exported to Great Britain (England, Scotland and Wales) must be accompanied by the relevant health certificate. Exporters should read the guide on how to complete a health certificate.

Certificate GBHC410

Must be used from 30 April 2024 by all countries.

Model health certificate for raw milk from third countries or parts thereof authorised in column A of Annex I intended for further processing before being used for human consumption (Regulation 605/2010) (Milk-RM).

Certificate GBHC411

Must be used from 30 April 2024 by all countries.

Model health certificate for dairy products derived from raw milk for human consumption, from third countries or parts thereof authorised in column A of Annex I (Regulation 605/2010) (Milk-RMP).

Certificate GBHC412

Must be used from 30 April 2024 by all countries.

Model health certificate for dairy products derived from milk of cows, ewes, goats and buffaloes for human consumption from third countries or parts thereof authorised in column B of Annex I (Regulation 605/2010) (Milk-HTB).

Certificate GBHC413

Must be used from 30 April 2024 by all countries.

Model health certificate for dairy products for human consumption from third countries or parts thereof authorised in column C of Annex I (Regulation 605/2010) (Milk-HTC).

Certificate GBHC414

Must be used from 30 April 2024 by all countries.

Model health certificate for colostrum of cows, ewes, goats and buffaloes and colostrum-based products derived from colostrum of the same species from third countries or parts thereof listed in column A of Annex I for human consumption (Regulation 605/2010) (C/CPB).

Certificate GBHC415

Must be used from 30 April 2024 by all countries.

Model health certificate for raw milk, colostrum, dairy products or colostrum-based products for human consumption, intended for transit or storage (Regulation 605/2010) (M/C/DP-T/S).

Certificate GBHC416

Model health certificate for dairy products and raw milk that have undergone a lower heat treatment than pasteurisation for human consumption from third countries listed in accordance with GB import authorisation IMP/GEN/2024/01

Certificate GBHC204

Use this certificate from 21 June 2021.

Model health certificate for milk and milk products from New Zealand.
