Downloading forms
We are aware that some users who select file links on this page are unable to download and save the forms.
While we investigate this, users can right-click a file link and select Save link as from the drop-down menu to download, save and edit the form.
Online FP17 forms will be unavailable from 8am on Saturday 31 October to 8am on Monday 2 November
This is due to planned downtime.

Compass is due to be replaced by the NHS Dental Services Portal (DSP).

Adding, removing, or amending clinicians - England only
To add or remove a clinician from your contract, or to amend their estimated earnings, you must:
- Make the change using the 'Contract Amend' function on Compass.
- Complete a Compass authorisation form (CAF) and email it to your commissioner.
You should submit a CAF within 7 working days of making the changes on Compass.
